[ID3 Dev] Revised chapter frame proposal

Colleagues,

Please find attached a third revision of the Chapter frame proposal. It incorporates some changes requested on the reflector over the last week and some clarifications.

In summary:

(1) The child elements of a CTOC can now be a combination of CTOC and CHAP frames, rather than one or the other. As a result, the Child Element Type flag has been removed.

(2) The semantics of the Top-level flag have been changed so that it now identifies a single, unique CTOC which is the root of the Table of Contents tree.

(3) Added text to explain that the Element ID should not be used as a human readable identifier.

(4) Added a recommendation that CTOC and CHAP frames should include a TIT2 sub-frame to provide a human readable identifier.

If you have any further comments please let me know. Hopefully, we're getting close to finalization.
